POSITION: Psychiatric Nurse Practitioner
Our Brookwood Health Center is seeking a mission minded full-time Psychiatric Nurse Practitioner to join the Santa Rosa Community Health Centers team. The Psychiatric Nurse Practitioner works in an interdisciplinary setting in collaboration with advance practice Clinicians and Mental Health staff. Services include diagnostic and prescribing services, community outreach, County case manager and line staff. The patient population includes adults suffering from homelessness, significant mental illness and chronic disease challenges stemming from a variety of socially determined health outcomes. Brookwood aims to close the gaps in health care disparities for our marginalized communities through compassion, commitment to excellence in health care services and strong community partnerships.
The Brookwood Health Center provides primary medical care and mental health services on a drop-in, same-day, and by-appointment basis. At Brookwood, we provide a wide array of medical, mental health, substance recovery, and case management services to meet the urgent needs of our most vulnerable members of the community. We also partner with Catholic Charities’ Nightingale respite program which provides housing for adults suffering from homelessness or severely limited social support recuperating from surgery or serious illness.
Job Summary: As a member of the Patient Centered Medical Home Team, the Psychiatric and Mental Health Nurse Practitioner, under the clinical supervision of the Chief Medical Officer and Director of Integrated Mental and Behavioral Health Services, assists in creating partnerships between individual patients seeking care at the Santa Rosa Community Health Centers and the providers by delivering excellent, comprehensive psychiatric treatment, primary health care, prevention, and health education.
Duties and Responsibilities:
Creates an ongoing relationship with each patient by taking complete medical history, taking complete psychiatric history, performing physical exams and making an assessment and diagnosis on adult and pediatric patients, individuals and families; initiating, reviewing, and revising treatment and therapy plans.
Diagnose, treat and dispense medication.
Assess, diagnose and treat emergent and chronic psychiatric and substance abuse related disorders, including crisis intervention.
Practice psychiatry in a community mental health delivery system.
Maintains adequate documentation of all patients seen, including follow-up and referral in the electronic medical record.
Manages the stable phases of chronic physical and mental illnesses in consultation with the attending physician, psychiatrist or Medical Director as needed.
Orders and interprets laboratory reports and diagnostic imaging reports.
May manage uncomplicated prenatal patients, complicated pregnancies when working under close physician supervision, and postpartum follow-up for mother and child.
Coordinates health and mental health management with physicians and other agencies.
Assures quality and safety by a care planning process, evidence-based medicine, and active participation of patients in decision-making.
Communicate with primary care physicians regarding mental health status of referred patients.
Provide consultation to primary care physicians and other mental/behavioral health staff.
Recognizes emerging community health and mental health problems and helps initiate appropriate interventions through community action.
May provide continuity of care through periodic home visits to bed-ridden and other patients.
Meets with the Medical Director or Attending Physician or Psychiatrist on a regular basis to review charting and medical and psychiatry care provided.
Seeks consultation on any difficult patient as described in the mid-level protocols in the Medical Administrative protocols Manual of SRCHC.
Participates in ongoing training.
Current unrestricted Psychiatric and Mental Health Nurse Practitioner License and Board Certification through the American Nurses Credentialing Center.
Current DEA license.
At least two years of experience in a community health center setting preferred.
At least two years of experience with psychotropic medication management required.
Ability to work independently with physician or psychiatrist telephone backup.
Current CPR certification.
Demonstrated proficiency with all tasks on the most current Psychiatric and Mental Health NP competency list.
Fluency in Spanish and English (speak, read, and write) preferred
