As a federally qualified health care center, Foothill Community Health Center (FCHC) provides primary and preventive healthcare services (medical, dental, and behavioral health) to the residents in the underserved area of East San Jose. The Substance Abuse Case Manager function is to facilitate substance abuse prevention services and treatment for our patients. The position offers case management support through providing addiction risk assessment, screening for mental health needs and offering additional addiction prevention and reduction resources as necessary.
In addition to Case Management responsibilities, the Clinician continues to provide individual and group counseling. S/he will conduct comprehensive assessments, treatment planning, risk assessment, clinical interventions, and case management. A Lead Clinician embodies the mission and philosophy of the organization to provide competent and comprehensive medical, dental, and behavioral health services to our community.
DUTIES AND RESPONSIBILITIES:
Provide screening and assessment for opioid treatment services.
Provide referrals to increase access to appropriate and accessible behavioral health treatment.
Provide referrals to substance abuse services focusing on the treatment, prevention, and awareness of opioid abuse.
Perform case management tasks (collaborating with medical providers, community service providers).
Increase patient access to medication assisted treatment (MAT).
Provides individual and group counseling services to children, adolescents, and adults at community clinic site at least two days per week and maintains an average of at least 7 encounters a day.
Attends monthly consultation group.
Stays up to date on all documentation requirements within the agency and legal/ethical guidelines.
Remains up to date and adheres to professional code of ethics.
Complies with state, county, and local laws governing the profession.
Seeks consultation from Clinical Supervisors if there are any ethical and/or legal concerns regarding a case.
Assesses and diagnoses clients using DSM 5, develops treatment plans, and occasionally helps manage on-site crises.
Utilizes electronic health record for daily charting.
Drives between clinic site locations and administrative office for meetings.
Must be able to travel and work at all applicable locations.
Availability to work all operational hours as needed.
Other tasks and projects as assigned by supervisor, manager and/or director.
EDUCATION AND/OR EXPERIENCE:
A Master’s degree in Social Work, Licensed Marriage and Family Therapist or similar field.
Current, valid California licensure from the Board of Behavioral Sciences. All licenses and registrations must be in good standing.
A minimum of 2 years’ experience working with substance use/abuse population.
A minimum of 2 years’ experience working with children, adolescents and/or adults in a counseling setting.
Training in domestic violence, child abuse, suicide and crisis management, and trauma-focused CBT.
Valid CA driver’s license, current vehicle registration, and proof of current automobile insurance.

Microsoft Word, Excel, and Outlook, web browsing, and electronic health record (EHR)
Courteous and professional in all interactions with clients, the community, and colleagues

Proactive in identifying potential problems as well as their solutions
Self-aware and keeps personal biases from interfering with job performance
Qualified as a Drug and Alcohol Counselor
Fluent in Spanish
Trained in SBIRT
Experience working with culturally diverse client populations.
