Chapa-De has an opportunity for a Nuse Case Manager for Substance Use Disorder to support their medical team at their clinic in Grass Valley, CA.
As Nurse Case Manager for Substance Use Disorders you will be responsible for collaborating with all Chapa-De providers and other staff to improve treatment outcomes for patients with substance use disorders. Additionally, this position supports data collection and data reporting to the grants that support Chapa-De’s Medication Assisted Treatment Program (MAT).
Example of Duties include:
Provide case management by collaborating with Chapa-De providers to improve treatment outcomes for patients with substance use disorders. This can include assessing patients’ needs for medication assisted treatment Patient counseling for SUDs: Recommending additional community resources Providing buprenorphine for opioid dependent patients with chronic pain Providing smoking cessation classes Providing Wellness (lifestyle change) and stress management classes
Knowledge of and experience working with patients with substance use disorders
Ability to interview and assess patients, using appropriate assessment tools, and observe, record and report on an individual’s functioning
Ability to communicate in a compassionate and professional manner with all patients and families
Ability to interact effectively with all members of the health care team
Ability to utilize multiple technologies for gathering data to meet required measures and to evaluate outcomes of specific programs.
