Altura Centers for Health (ALTURA) is a Federally Qualified Health Center (FQHC) in Central California that provides comprehensive, integrated, healthcare and social services to the underserved patient population in the San Joaquin Valley of California. ALTURA has been providing medical and dental care to the community since 1995, in response to community concerns regarding the health and social needs of low income immigrant population. ALTURA is a Patient Centered Medical Home level 3 that serves nearly 27,309 patients annually.
The Family/Internal Medicine Physician is responsible for the direct provision of primary care medical services while maintaining a high quality standard set by the policies and protocols of the department. Physician is further responsible for assuring an efficient and effective system in which quality medical care is provided to all patients. We are located in a rural area, but close to a suburban area. Tulare is an agricultural/dairy community, we (Tulare) hosts the World Ag Expo (http://www.internationalagricenter.com/2013-world-ag-expo) each year. Our patient demographic, include migrant farm works, medi-cal recipients and private insurance.
